I can't seem to get the end game to trigger. I have all the criteria you listed.

Edit: It suddenly triggered somehow. Not sure what I did differently.

Edit 2: The last ending doesn't really make sense. I don't want to give it away so I don't want to say too much but it's too similar to the second ending.
